System Navigator (Bilingual English/Spanish)
Position Overview:
The System Navigator supports families by coordinating health care navigation, medical payment resources, and intensive social services in collaboration with county providers. This role promotes Healthy Beginnings program goals, delivers safe sleep education to caregivers of infants, and provides field-based services with flexible scheduling to meet client needs.
Key Responsibilities:
- Engage families through home or community visits to assess needs and connect them to health, financial, and transportation resources.
- Assist clients with navigating county systems, completing medical forms, and reducing barriers to care.
- Serve as a liaison with community partners to coordinate services and streamline access.
- Maintain accurate documentation, data entry, and communication with supervisors to support program outcomes and compliance.
Qualifications:
- High School Diploma/GED required. Associates/Bachelor's degree preferred.
- One (1) year experience working with families and providing social services and community resources support in Palm Beach County.
- Bilingual English/Spanish required.
- Valid Florida’s driver’s license is required.
