Scheduler

Community Care Network is looking for a Scheduler to join our team!

About the Role:

The Community-Based Scheduler plays a critical role in coordinating services for individuals receiving Developmental Services (DS) supports in the community. This position is responsible for scheduling staff who provide employment supports and community-based activities, ensuring services are delivered efficiently while accounting for travel time, transportation needs, and individual support plans. The Scheduler supports continuity of care, promotes meaningful community engagement, and serves as a key communication link between individuals, families, staff, and supervisors.

Responsibilities:

  • Schedule Direct Support Professionals (DSPs), Job Coaches, and other community-based staff to support individuals with DS at their places of employment and in community activities.
  • Coordinate schedules that account for travel time, distance between locations, and transportation logistics.
  • Schedule and coordinate community-based activities in accordance with individual service plans and goals.
  • Adjust schedules in real time to respond to changes such as staff availability, transportation issues, or individual needs.
  • Maintain a daily and weekly schedule that ensures adequate coverage while minimizing travel inefficiencies.
  • Coordinate transportation supports, including pick-ups and drop-offs for individuals as needed.
  • Schedule staff time appropriately for transportation-only supports when required.
  • Communicate clearly with staff and individuals regarding pick-up times, locations, and schedule changes.
  • Track and adjust schedules to accommodate delays, cancellations, or community-based barriers (e.g., weather, traffic, job site changes).
  • Maintain accurate and up-to-date scheduling records in the Electronic Medical Record (EMR) and/or scheduling system.
  • Input new participant information and update service locations, transportation needs, and authorized support hours.
  • Maintain scheduling databases and complete required documentation related to services delivered.
  • Process referrals and schedule new individuals for community-based supports in collaboration with intake or program staff.
  • Ensure schedules align with authorized service hours and funding requirements.
  • Serve as a primary point of contact for individuals with DS, families, employers, and community partners regarding scheduling and transportation coordination.
  • Provide respectful, person-centered communication that supports choice, dignity, and independence.
  • Communicate scheduling conflicts, access issues, or staffing concerns to supervisors or program leadership.
  • Collaborate closely with supervisors and program staff to ensure services meet individual goals and community expectations.

Qualifications:

  • High School Diploma or equivalent; Associate’s Degree or relevant coursework preferred.
  • Previous experience in an administrative or scheduling role.
  • Attention to Detail, accuracy in maintaining records and managing data.
  • Team Player: Ability to work collaboratively across departments.
  • Knowledge and proficiency in the use of technology:  Microsoft products, software packages, EMR, etc.
  • Excellent communication skills and a client-centered attitude.
  • Must be comfortable working in a trauma-informed, culturally competent environment serving diverse populations.
  • Must have a valid driver’s license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • Successful completion of a criminal history background check after hire.
