Licensed Practical Nurse

This opportunity is for an LPN to work DAYTIME hours in an Outpatient Clinic.

 The LPN works closely with Psychiatrists in providing practical nursing duties. Assesses and observes the individual's emotional and physical response to medication and treatment. Communicates information to the individual's health care provider, their Pharmacy, families, caregivers, and other Providers involved with the individual. Communicates doctor's orders to individuals between appointments as needed. Provides direct psychiatric nursing care to individuals in order to provide individualized, goal-directed care. Assists individuals with treatment plan completion, obtaining vital signs, updating medications in the Electronic Health Record (E.H.R.) and assisting with medication refills as directed by the Psychiatrist.  Completes appropriate referrals as identified during the assessment process.  Perform other related duties as required.

Must be a graduate of an accredited practical/vocational nursing program and hold a current license as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.
