Psychiatric Mental Health Nurse Practitioner

Are you a compassionate, community-minded PMHNP ready to support patients through accessible, whole-person care? Southside Community Health Services (SCHS) in South Minneapolis is seeking a Psychiatric Mental Health Nurse Practitioner to provide assessment, diagnosis, and treatment in an integrated community clinic setting.

About the Role

This position provides psychiatric evaluations, medication management, and treatment planning for a diverse child and adult population. You’ll collaborate closely with primary care and behavioral health teams while supporting continuity of care, quality standards, and timely documentation.

Key Responsibilities

  • Provide psychiatric evaluation and medication management for children and adults
  • Conduct diagnostic assessments, treatment planning, psychotherapy, and discharge planning
  • Maintain an assigned caseload and ensure timely chart completion and encounter closure
  • Coordinate care with interdisciplinary teams and consult/refer as clinically appropriate
  • Participate in clinical protocols, quality improvement standards, and staff meetings
  • Provide patient education and support health education efforts
  • Assist with additional duties as assigned

Qualifications

  • Degree from an accredited school of nursing
  • RN plus national certification as a Psychiatric Mental Health Nurse Practitioner
  • Current Minnesota APRN registration and licensure
  • Bilingual (English/Spanish or Somali) preferred
