Care Manager
Job Summary
The Care Manager reports to the Senior Director of the designated campus. The Care Manager will serve as a liaison to external stakeholders, such as Recovery Court and other criminal justice entities, facilitating court appearances for clients in all residential programs, and transportation to and from court appearances and other assigned appointments. Additionally, the Care Manager shall be responsible for assessment and treatment planning for all clients in the campus. The Care Manager shall maintain exceptional organizational skills to manage sensitive client information, including clinical and legal and must prioritize competing demands such as urgent court appearances, time sensitive assessments, and other clinical documentation. The Care Manager will serve all residential programs in the assigned campus and may be relocated based on business needs.
This is a full-time, exempt position. The Care Manager will work traditional work hours Monday – Friday 9:00am-5:00pm, on-site. The Care Manager will be required to work earlier and/or later as needed to attend court appearances. The Care Manager will share in the holiday coverage of the campus as scheduled. The Care Manager must report in person to work.
Key Responsibilities
Stakeholder Liaison & Court Representation
- Serve as the primary liaison for with NJ Recovery Court & other legal stakeholders, collecting all patient progress prior to the meeting, sharing with Recovery Court prior to scheduled meeting.
- Document court appearance (virtual or in person), and all related services in the electronic health record
- Bridge communication between counseling staff and external partners by relaying clinical updates, treatment adherence, and any barriers to care.
- Ensure all Level of Care Index (LOCI) assessments and other required documents are submitted to the stakeholder in time for court appearances and/or continuing care, such as submission of all LOCI (extension requests) from counseling staff to the referring stakeholder where applicable.
- Provide physical transportation for clients to attend court using agency vehicles, as well as other appointments as needed.
- Attend all judiciary related trainings and other engagement events (i.e. client graduations, job fairs, etc.)
Assessment & Treatment Planning
- Conduct thorough biopsychosocial and risk assessments (during the 1st week of admission) for all clients upon admission to all residential programs in the campus.
- Formulate initial (within 72 hours and during the 2nd week of admission) and revised treatment plans (every 30 days) within the Department of Health regulations and CARF accreditation based on assessed needs, using a multidimension approach, all residential programs in the campus.
- Coordinate with counseling and medical staff, as well as other related disciplines as applicable, to communicate client driven goals and progress.
- Ensure all documentation is current, complete, and present in clinical record in a timely fashion.
- Engage in weekly clinical supervision related to assessment and treatment planning.
Care Management
- Act as a non-clinical resource, assisting with resource linkages for housing, employment, and medication management.
