Care Coordinator: Youth Diversion Case Management

The Youth Diversion Case Management (YDCM) Care Coordinator provides family-based case management support, skill building services and connection to community resources to children and families referred through Sonoma County Juvenile Probation Department. The YDCM Care Coordinator will provide flexible case management services within continuity of care contexts, including advocacy for services within the community, social service system, educational and health systems to reduce symptomatology and increase stabilization and functionality. Services will be provided almost exclusively in the community: in families’ homes, foster homes, schools, etc.

Responsibilities

  • Serve as Facilitator and work with treatment team to provide intake assessment and treatment planning, case management and skill building as needed at the site, in client homes, and in the community.
  • Facilitate and participate in the Child and Family Team meetings, treatment reviews, program staff meetings, and Individualzed Education Plans (IEP) meetings when necessary.
  • Provide consultation and care plan coordination to families.
  • Deliver culturally competent and sensitive services by discovering what is important and unique to each family and tailoring service location, language or focus to those needs. 
  • Work effectively and collaboratively with a variety of public/private providers.
  • Complete treatment plans, progress notes, monthly reports, discharge reports and other required paperwork in a timely manner.
  • Assist in data collection for further project development and research.
  • Maintain client charts and records to Utilization Review standards.
  • Maintain close communication with all treatment team members.
  • Participate in weekly individual and/or group supervision as required.
  • Attend and participate in all trainings as assigned.
  • Maintain professional behavior at all times, appropriately representing Seneca’s mission and philosophy.

Qualifications

  • Bachelor’s degree required
  • Must be eligible to be certified as a Mental Health Rehabilitation Specialist (MHRS), equivalent of 4 years full time experience in a mental health setting required
  • Bilingual in Spanish, a plus! 
  • At least two years of full-time experience directly providing support, community-based, or school-based services to children, adolescents and families is required
  • Own vehicle, valid California driver’s license and current automobile insurance
  • Clean driving record and must be insurable by Seneca’s insurance company
  • Clearance of TB test, physical exam, fingerprints, and any other State/Federal requirements
  • Proof of COVID-19 vaccination

Schedule

  • Full-time
  • Flexible schedule required, with the ability to work some evening and weekend hours as needed
