Bilingual Case Manager

The  Bilingual Case Manager position provides direct support to families within Orange County’s outpatient mental health clinics that have been referred by their assigned therapist or other service provider. These services include but are not limited to: referrals for community-based providers, support with navigating referral process, actively linking families to services, advocacy for services within the community, social services system, educational and health system of services, etc and follow up on outcome of resources.

We are honored to lead a nationwide effort, Todo Por Mi Familiaa campaign to find and connect families who sought asylum in the United States to mental health services. Seneca’s agency-wide DEI Advisory Board, Employee Resource Groups, and Language Practice Groups help our employees provide culturally and linguistically relevant services to the families we serve.


Responsibilities

  • Assess for and provide referrals for clients/families to improve overall well-being.
  • Support family in navigating referral process and problem solve barriers to access.
  • Consult with and coordinate with treatment team to ensure appropriate linkage and outcomes.
  • Work effectively and collaboratively with a variety of public/private providers.
  • Maintain appropriate record keeping and reporting requirements.
  • Assist in data collection for further program development and research.
  • Maintain a database of community partners/referral sources and update database ongoing.
  • Identify areas of need in the clinic program and work collaboratively with community partners to identify resources to meet these needs.
  • Offer direct services to client and family members by phone, telehealth or in person to promote access and linkage to referrals.
  • Follow up with families after referral is made to determine outcome and ongoing needs.

Qualifications

  • Bachelor’s Degree required, preferably in social work or related field.
  • Bilingual, Spanish speaking required.
  • Direct work experience providing support or community-based services to children, adolescents and families (preferred).
  • Strong computer skills
  • Valid California driver’s license and current automobile insurance
  • Clean driving record and must be insurable by Seneca’s insurance company
  • Clearance of TB test, physical exam, fingerprints, and any other State/Federal requirements
  • Proof of COVID-19 vaccination

Schedule

  • Full-time; Monday- Friday 9am - 5pm
  • Flexible schedule required with ability to work some evenings
