Clinical Care Coordinator, CP Program
CSO is looking for a Clinical Care Coordinator to join our Community Partner Program in Greenfield.
The Community Partner Program is an innovative service provider with a multidisciplinary team consisting of nurses, Licensed Clinical Behavioral Health Specialists, Bachelor's and Master's level Care Coordinators, and Peer Partners. Together we provide individualized support to adult MassHealth members who have complex medical and behavioral health needs. We coordinate communication between behavioral health, substance use, and medical care providers, and we connect people to services in the community to help them meet their self-identified health and wellness goals.
Responsibilities
- Provide telephone or other electronic outreach to assigned and referred Members, as cases are assigned, to determine whether they wish to enroll in CP services. Use Motivational Interviewing approach and other established engagement strategies to help Members engage.
- In collaboration with current primary care, behavioral health and other providers, develop a Comprehensive Assessment and establish a Care Team for each Member, supporting the care planning process.
- Facilitate the development of an individualized Care Plan for each Member, reflecting the input of all Care Team participants and the Member's wishes.
- Facilitate Care Plan Team meetings.
- Support development of natural support systems and community connections.
- Provide care coordination among providers, according to the Care Plan and Member's wishes.
- Make referrals and linkages to appropriate services along the continuum of care, consistent with the Care Plan.
- Develop and manage transition-of-care plans for Members discharged from inpatient care.
- Maintain regular contact with Members, in a manner and at a frequency that is consistent with the Care Plan and the Members' preferences and expectations.
- Provide wellness resources for Members.
- Assist members (and families, as appropriate) with system navigation.
- Work collaboratively with all members of Care Teams and CP teams.
Qualifications
Master's Degree in Psychology, Counseling, Social Work or closely related field from an accredited program. LICSW, LMHC, LMFT, LADC I, or LPN preferred. License eligible staff are expected to achieve licensure within 6 months of hire. Experience required in working with adults with SMI/SUD and in supervising nursing staff. Must possess skills in providing education and planning regarding treatment access and service needs, conflict resolution, risk management/safety planning, client advocacy and support. Good listening skills, nonjudgmental ability to develop staff strengths, capacity to create and lead an effective care team. Ability to establish trusting relationships through compassion, empathy and insight. Capacity to be an effective member of a Team. Demonstrated sensitivity to the needs and perspectives of individuals from diverse cultural or linguistic backgrounds. Computer proficiency required including ability to work with email, electronic health record and word processing software. MA driver's license and reliable transportation required.
