Clinical Case Manager

Salary Range: $69,300- $75,730 per year
A hiring bonus of $5,000 is available to external candidates with $2,500 paid upon hire and $2,500 paid upon completing 6 months of employment with St. Francis House. 

There is a tuition reimbursement program of $25,000 that will be paid in increments of $5,000 at the end of each year of employment for the first five years of employment. 

Job Summary:
Saint Francis House Housing and Stabilization team is seeking a Clinical Case Manager to work collaboratively and across departments to help provide direct assistance and support to formally homeless individual who have transitioned into permanent housing. The Clinical Case Manager will help the new residents develop and apply the skills learned to access resources needed to live successfully in a stable permanent housing environment. The Clinical Case Manager provides behavioral intervention, clinical consultation, and crisis intervention in support of residents who are actively substance using.

Essential Duties:
  • Welcomes new residents into housing and provides services and supports that foster a smooth transition from homelessness to a stable living environment in permanent supportive housing. 
  • Assesses the needs of individual residents through engagement and strength-based process and creates individualized service plans. 
  • Carries a caseload of approximately 10 individuals, whose needs will vary over time, but approximately 10% of which will have high levels of service needs at any given time.
  • Provides individual and group intervention around substance abuse and mental health.
  • Connects residents to treatment and service resources.
  • Utilizes best practice methods of clinical practice including motivational interviewing and harm reduction.
  • Provides services to residents that empower each individual and promote pathways to self-determination and stability, using an integrated “Universal Design” model of care that is Person-Centered, Recovery Focused and Trauma Informed. 
  • Maintains real-time, accurate records of residents’ assessments, case notes, and service plans in client database.
  • Provides group consultation to case managers around substance abuse and mental health of the residents.
  • Actively participates in individual supervision, team meetings, case conferences and presentations. 
  • Works collaboratively with SFH Workforce Development, Recovery Support Center, and other on-site services and community to develop, promote, and facilitate positive activities for residents living in St. Francis House housing communities. 
  • Adheres to agency code of conduct.

Requirements

  
Requirements:
  • Master’s degree in social work, counseling, or related field. 
  • CSW or LICSW preferred.
  • Minimum of two years providing direct service with vulnerable populations. Internship experience is considered.
  • Experience working directly with people experiencing substance use disorder and homelessness or who have recently exited homelessness, preferred.
  • Lived experience of homelessness and/or recovery a plus.
  • Driver’s license and regular access to reliable transportation strongly preferred.
  • Bilingual (Spanish), preferred.
