Job Description


Counselors support young adults incarcerated in NYC jails, specifically those with current substance use issues or those at-risk for developing a substance use issue, by providing jail-based support and coordination of services, court advocacy, discharge planning, and re-entry support. The position requires coordinating with various city agencies and non-profit organizations with the larger goal of reducing fragmentation and improving the outcomes of young people in NYC jails. The position focuses on working with high-risk young people, many of whom have been charged with violent felonies and who have high security classifications.

  • One-on-one meetings with clients 2-3 times/week to provide ongoing jail-based support.
  • Engage clients and assess the need for housing/employment/educational/health needs upon discharge from NYC jails and complete all necessary documents to facilitate this process, including follow-up to receipt of eligible services.
  • Provide court advocacy.
  • Work with incarcerated young people and other community-partners to facilitate peer-education groups.
  • Coordinate with jail-based services and other city agencies to reduce fragmentation of services.
  • Enter client information into appropriate electronic systems including electronic health record, and application submission systems.
  • Collaborate with other CHS Service team members to facilitate the delivery of services to the patient population through a mission-driven process that views services globally from incarceration to the community.
  • Participate in team meetings, unit activities; court/diversion advocacy and group interventions.

Salary Range: $46,425 - $60,000


Minimum Qualifications


1. Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or,

2. Education and/or experience equivalent to (1 above). Study at an accredited college in Sociology, Psychology or other behavioral sciences may be substituted on a year-for-year basis for the experience described in (1 above). However, all persons must have at least one year of full-time experience as described in (1 above).
