WISe Care Coordinator

The Wraparound with Intensive Services (WISe) program provides comprehensive behavior health supports to youth with complex behavioral and mental health needs. The goal of the program is to help youth thrive in their homes and communities, and assist in helping them meet the goals they have identified. The Care Coordinator coordinates services and planning for children and families that are referred and screened into the WISe program. The Care Coordinator is to guide, develop, implement, and monitor individual case plans and Child Family Team (CFT) ensure client and family voice. This role will guide their team through the WISe process and phases, and hold the central point of communication between the WISe team and client. Services will be provided almost exclusively in the community: in families’ homes, foster homes, schools, etc. The primary role is to ensure that all parties are working together toward stability for the child and family. Responsibilities include: direct services to children and families including care coordinating/linkage functions, maintenance of case records and progress notes, work as a part of a multidisciplinary team, flexibility to work evenings and weekends.

Responsibilities

  • Build, plan, facilitate and communicate Child and Family Team (CFT) meeting details with WISe team members, clients and families.
  • Maintain a caseload of 10 -15 clients.
  • Ensure to incorporate input and needs of the youth when forming the team.
  • Establish and sustain an effective team culture by inviting CFT members to propose, discuss, and accept ground rules for working together.
  • Overtime, work to transition facilitation to allow for youth and/or family member to be facilitator of their own meetings.
  • Cultivate and maintain relationships with community members and providers.
  • Utilize consensus-building techniques to meet the needs of the youth and family.
  • Provide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriate.
  • Maintain team focus on scope of work for the progress/movement toward transition, acknowledging and celebrate successes and transitions.
  • Stay abreast of all up-to-date Cross System Care Plans for each client and any other related documentation.
  • Participate in crisis response system.
  • Participate in individual and/or group supervision..
  • Attend all WISe required trainings as defined by the County.
  • Willing to respond to crisis situations and be part of a rotating on call system.
  • Commit to and engage in self-evaluation as well as organizational goals, strategies, and systemic practices regarding diversity, equity and inclusion.

Qualifications

  • Bachelor’s degree in Social Work/Psychology/Counseling.
  • Own vehicle; valid Washington Drivers License and insurability by Seneca's automobile insurance carrier.
  • Ability to work as part of and supervise a multidisciplinary team.
  • Strong coordination, organizational and writing skills.
  • Flexible schedule with ability to work evenings and weekends.
  • Bilingual skills preferred.
  • Clearance of TB test, fingerprints, and any other state or federal requirements.
  • Proof of COVID-19 vaccinations and booster.

Schedule

  • Full-time.
  • Mon-Fri. Flexibility with daily schedule required.

  • Starting salary at $21.00- $23.50 hourly, actual salary is dependent on creditable experience above the minimum qualifications for the role
