Clinical Licensed Community Service Provider

Licensed Clinical Community Service Providers – Full Time 

Northern Rivers Family of Services is pleased to provide Children and Family Treatment Support Services (CFTSS). These mental health and substance use services, available with New York State Children’s Medicaid program, give children (up to age 21) and their families the power to improve their health, well-being and quality of life. This program is intended to offer opportunities to better meet the behavioral health needs of children early on and to prevent the onset or progression of behavioral health conditions. These services strengthen children and their families, and help them to make informed decisions about their care and are provided either at home or in the community.
 
We are seeking to hire multiple Full Time Licensed Clinical Community Service Providers in the Capital Region and beyond! These positions offer flexible schedules and incorporate both telehealth and in-person counseling sessions to meet the needs of our client children and families, while working with them in the comfort of their own homes and allowing the provider to establish a more personal relationship with the client. 

We have openings for Licensed Clinical Community Service Providers in the following counties: Albany, Clinton, Columbia, Delaware, Essex, Franklin, Fulton, Greene, Hamilton, Herkimer, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Ulster, Warren and Washington. 

Responsibilities of a Licensed Clinical Community Service Provider
  • Provide other licensed practitioner (OLP) services within the CFTSS array, which includes Licensed Evaluation (Assessment), Treatment Planning, Psychotherapy (Counseling) and Crisis Intervention Activities, based on the child and family/caregivers needs and preferences as identified in the Treatment Plan. Currently providing services through both in-person and telehealth visits. 
  • Complete an assessment of needs in the recommendation of further medically necessary services, such as rehabilitation services. Services are delivered in a trauma informed, culturally and linguistically competent manner.  
  • Work with family members, advocates and other salient parties to identify goals and objectives of treatment. Provide observations and information to collateral contacts for the purpose of the progress and tracking of the Treatment Plan goals. 
  • Complete all required documentation for submission to the Supervisor within the required timeframes.  

Education and/or Experience
  • Masters degree in Social Work, Counseling, Psychology or related field
  • 1 year of related experience
Certification / Licensure
  • NYS Professional Licensure/Limited Permit - LMFT, LMSW, LMHC, LCSW, CAT
  • NYS drivers license with a reliable vehicle, as work requires up to 50% community travel/client home visits
