In addition to experienced candidates, this is a great opportunity for recent Bachelor Level Human Services graduates looking to gain valuable experience! 
Your schedule will be 37.5 hours/week, Monday through Friday. Hours are flexible and includes remote work.
What You'll Do:   
  • Work with children, adults and their families who have requested access to services within the intellectual disabilities and mental health service systems
  • Carry a caseload of about 38 individuals who you will be able to develop strong relationships with over time
  • Locate information about the individual's needs and preferences, identify relevant services and supports, facilitate and coordinate connections to those services and supports, and monitor the individual's circumstances and changing needs over time  
What We Require:   
  • Bachelor's degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science
  • Valid PA Driver's License and the ability to travel throughout Bucks and surrounding counties
  • Computer proficient
  • Ability to perform the physical requirements of the job description, with or without accommodation
