Hourly Pay Range: $21.907203 - $27.569584

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS
The Clackamas County Health Centers Division is looking for a motivated, team-oriented Certified Medical Assistant to coordinate patient care and provide exceptional service to the community.

Clackamas County Health Centers offers health care services for families and individuals in Clackamas County. Staff provide high quality medical care - from physical exams to family planning. We are committed to using the latest in medical practices that focus on each patient as an individual, and we work to take care of the health and wellness of the whole person.

The Patient Care Coordinator will coordinate care for Clackamas County Health Centers' clients through an established and centralized process, including the coordination of care transitions; researching and screening of patient healthcare needs; outreach and direct contact with patients; engagement and coordination with stakeholders; and participation in project and initiatives aimed to improve care and efficiencies for the Clackamas County Health Centers.

The Patient Care Coordinator has one of the most important roles of a patient's journey towards health. The individual in this position must have excellent customer services skills, devoted to patient-centered care and have the ability to work in a fast-paced environment. This is an exciting opportunity to have some balance between desk work and direct service care, while working with a wide-range of skilled professionals and serve a very diverse group of patients.

CMAs provide patient education, record vital statistics, bandage, handle specimens, administer injections, chart documentation in Electronic Medical Record (EMR), as well as answer clinical calls, communicate health information and schedule treatments and follow-up appointments.

As a member of the team, you must maintain a caring, warm and supportive attitude towards patients, family members, visitors, providers and co-workers.

Required Minimum Qualifications/ Transferrable Skills:*

• Any combination of relevant experience, education and/or training that would likely provide the required knowledge and skills to perform the duties of the position
• Current certification/registration as a Medical Assistant by a nationally recognized program. (National certification programs include: American Association of Medical Assistants; National Certified Medical Assistant; American Medical Technologists)
• Exceptional skills and experience in customer service
• Experience with an electronic record-keeping system
• Experience working in a medical setting or healthcare setting
• Computer proficiency with Microsoft Office (ie: Word, Excel, Teams, Outlook)
• Basic Life Support (BLS) certificate

TYPICAL TASKS

• Care Transition Coordinator and Follow-up: Responsible for ED pre-manage reports and follow-up after ER/hospital visits; ensures the patient's charts are up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health. Responsible for verifying current insurance for assignment and establishing care for newly assigned patients.
• Pre-Visit Screening: Review patient history prior to scheduled care visit; adjust electronic health record in coordinator with Primary Care Provider and care team; ensure patient Health Maintenance listings are completed and up-to-date as outlined within Health Center's policy and processes; audit completion of needed Health Maintenance interventions, and report to clinical leadership.
• Patient Outreach: Responsible for making direct contact, via telephone or in writing, with patients and clients to engage them in services, inform them on health education subjects, motivate them to participate in screenings or health care activities, or follow up on past care; track and monitor completion of outreach and outcomes; schedule appointments and services as appropriate.
• Liaison with stakeholders: Coordinate with internal and external stakeholders in Clackamas County Health Centers care coordination activities.
• Participate in projects that are identified to improve quality of care, and/or efficiency of services.
• Clinical Builder: Improve workflows and develop procedures for documentation standards by clinical staff; EPIC Electronic Health Record optimization
• Social Determinant of Health Service Coordination: Refer and coordinate with organizations who provide services in social determinant of health needs through the UniteUS platform.
• Onboarding New Clinicians: Responsible for the introduction of tools to use within the Electronic Health Record and how to maintain a record with the most current information.

WORK SCHEDULE
This position works 40 hours during a standard workweek of Monday through Friday. Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington.
