Independent Living – Long-Term Support and Services Coordinator (IL LTSS)
The Center for Living & Working is currently seeking candidates for our Independent Living – Long-Term Support and Services Coordinator (IL LTSS) position. The person in this position is responsible for:
- Acting as an independent resource to dually eligible individuals (Medicaid and Medicare) between the ages of 21-64 enrolled in a One Care Organization.
- Assisting with the coordination of Enrollees’ long-term supports and services (LTSS) needs.
- Acting as an independent facilitator and liaison between Enrollees, the One Care Organizations, and service providers.
- Advocating for and with Enrollees regarding his/her LTSS needs.
- Educating Enrollees and his/her Interdisciplinary Care Team on LTSS.
- Arranging and coordinating the provision of community LTSS.
- Monitoring the provision and outcomes of LTSS including consumer determination of satisfaction with LTSS.
- Providing face-to-face IL-LTSS Coordination when Enrollees or Plan requests a face-face visit with IL-LTSS Coordinator.
- Fully participating in the initial assessment and development of a care plan to ensure that all relevant aspects of consumers care are addressed in a fully integrated manner and on an ongoing basis.
- Recording and collecting service authorizations for service provision and billing.
- Other duties as assigned.
Essential Functions:
- Frequent communication (orally and/or in writing) with consumers, staff, medical providers, state agencies, business partners/vendors and other third parties.
- Understand the requirements of the One Care Program and other community services and to be able to assess, educate, direct and refer the consumer/guardian regarding the scope and responsibilities of the programs.
- Travel within service area and beyond as needed.
- Accurately enter information into database.
- Use office machinery, including calculators.
- Maintain strict confidentiality.
- Participate in creating and maintaining an individualized care plan.
Qualifications:
- Bachelor’s Degree in related field such as social work or human services.
- 5 years of relevant experience may be substituted for educational requirements
- Minimum of 2-4 years of experience in human services.
- Working with people with disabilities, behavioral health needs or elders in need of LTSS
- Teaching Independent Living skills and providing advocacy while promoting self-empowerment
- Completion of person-centered planning and person-centered direction training.
- Knowledge of the home and community-based service system and how to access and arrange for services.
- Conducting LTSS needs assessments and monitoring LTSS delivery
- Cultural competency and the ability to provide informed advocacy
- Ability to write an individualized care plan and communicate effectively both verbally and in writing across complicated service and support systems

- Independent Living issues and philosophy.
- Community resources and local service system for persons with disabilities and how to arrange for services.
- MassHealth, state agencies and provider service system available to serve individuals with disabilities/ (older persons).
- Excel, Microsoft Office, data base management
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Remain in a stationary position for extended periods of time.
- Must be able to lift up to 15 pounds at times.
Job Type:
- Full Time
- Hybrid-remote. Must be present in the office at least 1 day per week.
