                                      SIGN-ON BONUS: $4000
Due to expansion within a large multi-agency effort to significantly reduce chronic homelessness in the City of Boston and surrounding areas, Eliot's Homeless Services Division is seeking several creative and strengths based Housing Coordinators.
We need creative and strengths based Housing Coordinators to join our CSPECH team. The successful candidates will have experience working with people experiencing homelessness, medical complexity, and/or untreated behavioral health concerns (e.g. PTSD). Working knowledge of area resources/providers is a plus. Recovery Coaches with case management experience will be considered for this role.
Responsibilities:
  • Ensure that consumers are treated with dignity and respect in accordance with ECHS’ Human Rights Policy.
  • Engage people served within a client centered/harm reduction/Housing First framework. make collateral contacts, and consult with other providers (i.e., mental health, primary care, etc.). Ensure that support services and interventions are person centered/strength based/recovery oriented
  • Collaboratively develop and implement effective housing navigation plan to cultivate pathways to short/long term sustainable housing with people served.
  •  Forge and maintain positive and productive working relationships with community-based stakeholders and coalitions. Respond to any landlord concerns as quickly as possible, seeking consultation from supervisor as needed.  
  • Conduct assessments with E-Hana and HMIS as needed, make collateral contacts, and consult with other providers (i.e., behavioral health, ACO, primary/specialty care, etc.). Conduct assessments for coordinated entry, income, and functionality. Complete housing applications, and obtain vital documents as needed/assigned.  
  • Provide long term/ongoing tenancy sustaining stabilization services once person served is housed. Support plan will address clinical, medical, and safety concerns.  
  • Recognize and respond in real time to sudden changes in client behavior, physical health and/or related medical issues, or other related areas of concern.  Seek consultation from multidisciplinary team and/or direct supervisor as needed. Develop and implement a crisis response plan with consultation and support from supervisor as needed to ensure safety. 
Qualifications:
  • Requires a Bachelor’s degree in a human services field; social work, psychology, human services, or sociology preferred – or 2 years case management or comparable experience with the homeless population.   
  • A Master’s degree in the healing arts is strongly preferred.  
  • Knowledge/experience in working with homeless adults with complex behavioral health and/or medical profiles including substance abuse or dependence.  
  • Knowledge of Public Entitlements (EAEDC, Social Security, Food stamps, etc.,) and Housing Continuums of Care (COCs) and ability to effectively leverage them to address clinical and systemic barriers to housing. 
  • Ability to assess client support systems, collaboratively develop housing navigation plans that incorporate support services that address identified treatment needs and preferences, and provide ongoing support stabilization services until tenancy sustaining services are established for newly housed individuals. 
Schedule: Monday-Friday, 8am-4pm
Bachelors; $48,000 Masters: $50,000
Spanish/English bilingual is required, you receive an extra 6%
