Summary: Participates as a vital part of a flexible, client centered team, to coordinate and provide transportation services for adults with prolonged mental or co-occurring illnesses. Assists clients and Rehabilitation services in achieving the goals of increased integration of clients in the community and building a strong social network.

Essential Responsibilities and Target Outcomes:

• Performs assorted pick-up, transportation, and delivery of services according to weekly transportation plan as assigned at appointed time.

• Maintains each assigned agency vehicle in acceptable working order in clean, sanitized condition, in accordance with CMHA vehicle maintenance standards.

Reports to Supervisor when any vehicle repair work is needed.

• Provides Monthly Vehicle Inspection Report by the 10th calendar day of each month.

• Submits weekly data to Managed Information System documenting client attendance for consumer-run, community-integrated activities.

• Reports and coordinates with Supervisor immediately when an unexpected event occurs involving a client or transportation-related incident or accident.

• Delivers interoffice mail, cash and makes special deliveries as assigned.

• Performs other related duties as assigned.

Education: High School Diploma or GED preferred.

$17-19 per hour
