Summary: The Housing/HMIS Data Specialist has responsibility for oversight of program-specific reporting and data requirements in accordance with policies, procedures, internal processes and external regulatory agencies including but not limited to HUD, DPH, DMHAS, and JCAHO. Maintains adherence to fidelity requirements. Monitors departmental budget and makes recommendations for future planning and growth.

Essential Responsibilities and Target Outcomes:
  • Ensure correct and timely completion of housing assessment, inspections and HMIS entries.
  • Identifies and analyzes systemic problems and acts decisively to implement solutions and resolve crises through appropriate referrals through the Coordinated Access Network.
  • Works collaboratively with the team members to ensure compliance with the voucher requirements.
  • Ensures timely and accurate documentation is maintained in accordance with CMHA policy and procedures.
  • Provides direct client care to complete required assessments.
  • Performs other related duties as assigned.

Minimum Requirements:
Education: Bachelor’s Degree in behavioral health (psychology, social work, counseling, marriage and family therapy, alcohol and drug counseling), required.

Credentials and Licenses: Valid Connecticut driver’s license

Experience:
  • Successful experience working with co-occurring homeless population, preferably a minimum of two years.
  • Intermediate or advanced experience with Data Entry systems.
  • Experience in, or the ability to learn how to use an Electronic Medical Record (EMR) system which includes appointment scheduling, authorization tracking, assessments, treatment plans, progress notes and discharge summaries.
  • Experience with HMIS, CAN, and 211 system
