Assistant Coordinator, Transitions

Job Description

The mission of the Boston Public Health Commission is to work in partnership with communities to protect and promote the health and well-being of all Boston residents, especially those impacted by racism and systemic inequities.

DUTIES

  • Facilitates groups and "one on one" counseling sessions.
  • Provides case management services to clients.
  • Obtains comprehensive psycho-social histories using a team approach.
  • Formulates treatment and discharge plans.
  • Maintains case management files, statistical information and client records.
  • Provides crisis intervention and supportive counseling to program clients.
  • Monitors client adherence to treatment program rules and regulations.
  • Assists staff in the management of environment and guest safety.
  • Attends assigned meetings and trainings.
  • Performs other duties as required.

MINIMUM QUALIFICATIONS

  • Bachelor's degree with minimum one (1) year related experience. College level course work equivalent to an Associate degree with two (2) years human service experience. High School diploma / GED and three (3) years experience may be substituted for entrance requirement.
  • CAC or CADAC preferred.
  • Previous experience working with homeless, diverse ethnic racial and low income population with understanding of mental health substance abuse and recovery issues.
  • Strong written and verbal communication skills. Computer skills a plus.

City of Boston Residency Required. A Criminal Offenders Records Information request must be completed for this position. However, a record is not an automatic bar to employment but is reviewed in relation to the job applied for., This position requires direct client/patient contact and as a result of such direct contact, certain immunizations will be recommended and/or required prior to commencement of employment duties., Any position that requires an advanced degree will be subject to education verification, This is a grant funded position, The Boston Public Health Commission is an EEO Employer and all applicants meeting the minimum requirements are eligible to apply.
