New Castle County Hope Center Case Manager

The New Castle County Hope Center seeks one (1) onsite Case Manager to offer Supportive Services & Case Management to individuals and families. These services will be offered during the Hope Center’s normal hours of operation, Monday to Friday, beginning at 7:00 am and ending at 7:00 pm, or otherwise agreed upon by the Parties. The hourly rate is $26.00.  All work is performed at the New Castle County Hope Center and could include both on-site and off-site clientele. 

 

Case Managers are responsible for working collaboratively with other Hope Center Case Managers, Hope Center Case Mentor/Monitors and their Supervisors, Outside Agency Case Managers, other HOME ARP Consultants, Family Promise of Northern New Castle County’s Rapid Re-Housing Case Management Staff, the Hope Center Managing Director and Assistant Director, aiding both families and singles transitioning out of homelessness and those populations meeting the eligibility requirements under the HOME ARP Funding Grant. Additionally, funding for this service may come from an array of other federal and non-federal sources. 

 

The Case Manager will provide housing navigation services, referrals to Rapid Re-Housing programs and linkages to New Castle County’s HOME ARPA Supportive Services Funding (security deposits, 1st month’s rent, utility payments, rental assistance).

 

HOME-ARP qualifying populations:

  • Homeless, as defined in section 103(a) of the McKinney-Vento Homeless Assistance Act (42 U.S.C. 11302(a));
  • At-risk of homelessness, as defined in section 401(1) of the McKinney-Vento Homeless Assistance Act (42 U.S.C. 11360(1));
  • Fleeing, or attempting to flee, domestic violence, dating violence, sexual assault, stalking, or human trafficking, as defined by the Secretary.
  • In other populations where providing supportive services or assistance under section 212(a) of the Act (42 U.S.C. 12742(a)) would prevent the family’s homelessness or would serve those with the greatest risk of housing instability;
  • Veterans and families that include a veteran family member that meet one of the preceding criteria.

The Hope Center Case Manager duties are as follows: 

 

  • Providing case management and supportive services to those qualifying populations outlined above; use of the Community Management (see below for entire outline of Case Management duties). 
  • Enhancing coordination with Hope Center partners, and outside agencies to reduce the barriers to being rehoused; and 
  • Working closely with Hope Center Management team on special projects and other duties as assigned. 
  • Enforcing the Hope Center Rent Readiness Program and monitoring guests progress to ensure they’re achieving the goals of becoming a highly “sought-after” tenant. . Does not cause damage to the premises. Keeps the property clean. Is respectful of neighbors and lets property managers know when problems arise.

Additional Case Management Duties Include, but are not limited to:

 

  1. Conducting the initial evaluation, including verifying and documenting eligibility, for individuals and families applying for supportive services.
  2. Counseling.
  3. Developing, securing, and coordinating services.
  4. Using a centralized or coordinated assessment system that complies with the requirements of Section IV. C of the HOME ARP Notice.
  5. Obtaining federal, State, and local benefits.
  6. Monitoring and evaluating program participant progress.
  7. Providing information and referrals to other providers.
  8. Providing ongoing risk assessment and safety planning with victims of domestic violence, dating violence, sexual assault, stalking, and human trafficking.
  9. Developing an individualized housing and service plan, including planning a path to permanent housing stability; and
  10. Conducting re-evaluations of the program participant's eligibility and the types and amounts of assistance the program participant needs.

 

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES: Good knowledge of business mathematics and English; some knowledge of community resource and social service agencies and their functions; ability to interpret and follow applicable laws, regulations, and policies; ability to communicate courteously and effectively, both verbally and in writing; ability to conduct interviews of a sensitive nature; ability to establish and maintain effective working relationships with  applicants and program participants, social service agencies, and the public; ability to handle emergency situations; ability to make mathematical calculations rapidly and accurately; ability to maintain accurate records and to compile and record data. 

 

MINIMUM QUALIFICATIONS: At least three (3) years’ experience in the social service field which includes working with program clients and possession of a bachelor’s degree from an accredited college or university with major course work in the social or behavioral sciences or related field; or an equivalent combination of education, experience or training directly related to the required knowledge, skills, and abilities.

 

ADDITIONAL REQUIREMENTS: Possession of a valid Delaware Class D driver's license or its equivalent. Must pass a Class III County physical examination, drug screening and background check.
