Access Point Care Coordinator, CCBHC IA

Position Summary: The Access Point Care Coordinator will work collaboratively as part of the CCBHC team to support overall wellness of referred patients. The care coordinator will assist with a patient's recovery efforts through addressing/ameliorating social determinant of health barriers by connecting with appropriate referral agencies and wrap around services.
Qualifications:
- Associates or Bachelors degree preferred, High School Diploma required.
- An understanding of local Rhode Island resources
- Bilingual required.
- Cultural sensitivity necessary to work with a diverse patient population.
- Possession of a valid drivers license, reliable transportation, registration and auto insurance is also required.
- Bilingual skills are compensated!!
