CASE COORDINATOR/FAMILY ADVOCATE of School Services

Salary Range: $50,872.50-$58,172.50. 

 

The SRT team is looking for a Family Advocate (FA) to join our Staten Island SRT team. The Family Advocate, through collaboration with SRT clinician, provides supportive services to enrolled children and families through linking families to the best fitting services and supports. To do this, the FA will build partnerships in the local community and have a knowledge base about resources in the community and across child service systems.

Responsibilities

The New York Foundling’s School Response Team (SRT) partners with schools around NYC to increase accessibility of mental health services to families by bringing high quality, evidence-based mental health treatment inside schools or linking students with the needed services in the community. We also aim to build school’s capacity to meet the social and emotional needs of their students by empowering schools to think and operate proactively and relationally through ongoing collaboration, school faculty consultations, and delivery workshops to the school community.

 

The SRT team is looking for a Family Advocate (FA) to join our Staten Island SRT team. The Family Advocate, through collaboration with SRT clinician, provides supportive services to enrolled children and families through linking families to the best fitting services and supports. To do this, the FA will build partnerships in the local community and have a knowledge base about resources in the community and across child service systems.

 

The FA will:

  • Engage primary caregiver and other key participants in order to complete intake to identify needed resources
  • Support in linking families to needed resources
  • Promote engagement of families and other key stakeholders (school staff, community agencies) through involvement coordination of care
  • Maintain effective working relationships and collaboration with school staff, agency staff, and community provider staff, including participation in community outreach.
  • Research, organize, maintain, and disseminate community resource information. Maintains current data base of community resources.  Maintains up to date knowledge of resources in Staten Island and New York City.
  • Enhance capacity of the school community (faculty and parents) to meet the social and emotional needs of their students through delivery of workshops in collaboration with clinician.
  • Participate in all training, supervision and consultation activities.
  • Able to run groups in collaboration with clinician
  • Ensure compliance with DOHMH and agency documentation standards by maintaining clear, concise, accurate, and timely clinical-approved documentation of treatment efforts
  • Flexible with participating in on-call rotation.

Salary Range: $50,872.50 - $58,172.50

Qualifications

Required Educational Level:

  • Bachelors’ Degree of Social Work, Psychology, or Sociology.

 

Other Requirements

  • Minimum of 1-2 years experience with case coordination
  • Knowledge of community and community resources
  • Excellent communication skills
  • Spanish skills preferred
  • Be proficient in Microsoft Word, Excel, PowerPoint, Internet, Email, and Data entry systems
  • Ability to work cooperatively to ensure the best quality services are recommended
  • Will be able to build and collaborate with community resource stakeholders

 

 

Education Required

Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
