Patient Navigator – Bridge2Quit/Patient Portal 

Location: Jamaica, Queens

Start Date: June 1, 2024

 

General Job Statement:

The Patient Navigator actively works to recruit English and Spanish speakers to enroll in their health center’s online patient portal as part of a large health services research project. The Patient Navigator will educate patients about the benefits of patient portal enrollment, address knowledge or barriers and assist with computer literacy issues.  This is an in-person position with a 30-hour workweek and pro-rated benefits. Candidates must live in the NYC region and be able to travel to their assigned health center daily. 

 

Requirements:

  • College degree or greater
  • Spanish fluency required
  • Experience working with vulnerable populations, including ethnic and racial minority communities
  • Strong communication, organizational, and PC skills
  • Ability to attend in-person training sessions
  • Ability to complete Human Subjects Research training
  • Successfully complete background check and medical clearance

 

Key Responsibilities:

  • Patient enrollment
    • Using an eligible patient list, establish rapport with patients, and engage to complete enrollment in the Patient Portal (Spanish or English)
    • Educate patients on the benefits of the Patient Portal and how to use it
    • Establish relationships with FQHC staff for warm hand-offs
  • Documentation & Reporting
    • Maintain continual documentation of activities, action plans, and results while adhering to the policies and procedures in the approved protocol
    • Collect data and complete relevant data entry in study database
    • Report adverse events
    • Participate in weekly project team meetings
  • Other duties as assigned
