PAY AND BENEFITS

 

Annual Pay Range: $87,447.48- $115,121.91

 

Hourly Pay Range: $42.042059 - $55.347073

 

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

 

This position is classified as a Policy, Performance & Research Analyst Senior, working title is Population Health Manager.

JOB DETAILS AND QUALIFICATIONS

 

Clackamas County Health Centers is looking for a team-oriented, visionary, and strategic leader who is passionate about the quality, health and well-being of the community. We are seeking a manager who can lead, support, and develop employees in establishing policy, system and program changes that allow residents of Clackamas County to have the opportunity to live, work, and play in a healthy, clean, and safe community.

 

The Population Health Manager leads the Population Health team by guiding the development, implementation, and monitoring of population health programs and quality improvement activities to improve clinical quality, reduce inequities and advance value-based performance and clinical quality goals for the Health Centers.

 

The Population Health Manager will plan, organize and coordinate the population health program for Clackamas Health Centers and responsibilities include, but are not limited to, supervising patient care coordinators, providing training and direction to staff who perform population health activities, advancing value based payment initiatives, improving performance on quality initiatives and reporting to clinical, quality and operational leadership about progress on organizational goals.

 

The Population Health Manager is a member of the Quality Improvement and Quality Assurance Team, and reports to the Deputy Medical Director, Quality.

 

Required Minimum Qualifications/ Transferrable Skills:*

 

• A minimum of five (5) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position.

• Experience in successfully leading and/or supervising a diverse team.

• Project management experience.

• Experience with community health surveillance and data interpretation.

• Experience in evidence based program planning, development, monitoring, reporting and evaluation.

• Experience in managing grant funded programs and projects.

• Experience in the health care field.

• Background in quality improvement principles and leading quality improvement initiatives

Preferred Special Qualifications/ Transferrable Skills:*

 

• Experience in a Federally Qualified Health Center (FQHC)

• System experience with EPIC or any EHR

• Degree in a healthcare field

• RN background

TYPICAL TASKS

 

Population health quality oversight: Coordinate with the Lead Data Analyst and Lead EHR Systems Analyst to develop and utilize population health tools including dashboards, rosters and internally and externally created data reports to track and improve performance on population health metrics; lead QI analyst population health initiatives, provide updates to clinical, quality and operational leadership on strategic initiatives and assist with setting strategies to achieve population health goals; develop and lead strategies to maximize effectiveness and efficiency of Health Centers population health activities.

 

Supervision of population health staff: Supervisor or provide joint-oversight with RN supervisors of allied health staff performing outreach, care coordination, chart scrubbing and other clinical support activities to close care gaps and provide preventive care supports.

 

Staff development: Develop and facilitate trainings and onboarding for staff on population health programs and workflows.

 

Quality and process improvement: Use improvement tools and methods including PDSA cycles in day to day work; perform and oversee clinical improvement activities to improve performance of teams; develop and participate in audits as necessary.

 

Project management: Manage population health projects to satisfy grant and regulatory requirements to improve value based care and population health metrics and initiatives; compile and analyze data to meet contractual financial and clinical obligations including administering budgets, adhering to contract reporting obligations

 

Communication and collaboration: Establish and maintain liaison relationships with personnel engaged in similar work with other local programs and agencies, including senior level staff, Clinic Managers and Supervisors; prepare reports to summarize clinical and operational performance for program areas and present to internal and external stakeholders. Serve on various staff committees, teams, and workgroups as appropriate.

 

REQUIRED KNOWLEDGE AND SKILLS

 

Knowledge of: Population health program theory, development, and practices; and

 

quality improvement theory, strategy, and practical methods to achieve rapid-cycle improvement.

 

Ability to: Supervise, motivate, lead, inspire, plan and direct the work of support personnel including clinical staff;

 

establish and maintain effective working relationships with clinical staff, quality staff and leadership staff, and with external partners;

 

compile and analyze data to meet contractual financial and clinical obligations including administering budgets and adhering to value based reporting obligations;

 

develop training materials including workflows, presentations and directions to support population health initiatives;

 

effectively communicate in writing and verbally;

 

creatively approach systems development and improvement while managing multiple priorities;

 

develop procedures and processes to meet population health objectives; and

 

effectively, under pressure, maintain focus on daily responsibilities for long-term

 

WORK SCHEDULE

 

This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

 

This position is eligible for hybrid on-site/telework subject to the https://dochub.clackamas.us/documents/drupal/9065b5fb-2237-47ce-9534-e7c8c40fc2bc and based on the Department's business needs and any orientation or training period.

 

The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
