EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Veteran Case Manager to work full time for its Queens Misdemeanor Treatment Veteran's Court Program.

The Veteran Case Manager position is a non-exempt position paying $28 per hour at 35 hours per week. The hours of operation are Monday-Friday 9:00AM-5:00PM and the office is located in Kew Gardens, NY. The role reports to Program Director.

Primary Purpose of Job:

The Veteran Case Manager is responsible for the assessment and case management of Veterans in the QMVTC program.

 

Principal Duties & Responsibilities:

  1. Conduct assessment interviews including referral for further evaluation/ services if needed.
  2. Complete treatment plan and present to the Court.
  3. Arrange for and implement placement into an appropriate treatment program.
  4. Assist clients in obtaining benefits as indicated, including public assistance, SSD/SSI, housing, medical and other needs.
  5. Maintain complete client records and have available for inspection by Supervision at any time.
  6. Escort clients to treatment facilities when indicated.
  7. Maintain caseload through ongoing client contact.
  8. Conduct Seeking Safety groups.
  9. Make appropriate and timely site visits to all treatment agencies as required.
  10. Assist with supervised toxicology testing.

 

Knowledge, Skills, & Abilities Required:

  1. Bachelor's Degree in Psychology or related mental health field or Behavioral Science required, or High School Diploma/educational equivalent required and a significant combination of education and experience in substance abuse and mental health.
  2. Must have a working knowledge of treatment programs and the Veterans Administration.
  3. Good communication skills, both oral and written. Ability to work cooperatively with treatment and mental health systems.
  4. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
