Job Summary

Reporting to the Supervisor of Integrated Care the Hospital Clinical Liaison serves as the primary point of contact for collaborative services with hospital partners; acting to enhance continuity of care for shared clients.  As a clinical provider, the Hospital Clinical Liaison will provide on-site, mental health assessments to newly referred clients and secures mental health services post discharge as appropriate.  Additionally, the Hospital Clinical Liaison will work collaboratively with hospital staff to establish relationships, workflows, and systems for effective communication.

 Job Qualifications

  • Must be actively licensed by the State of Ohio Counselor, Social Worker and Marriage and Family Therapist Board.
  • Prior experience in administering diagnostic assessments and utilizing various mental health-screening tools strongly desired.
  • Must have an awareness of and sensitivity to the cultural diversity of the populations served.
  • Must have a valid driver’s license in the State of Ohio and access to an insured vehicle

Essential Job Duties &Responsibilities

  • Administers mental health assessments to hospitalized patients that may present with a range of mental health symptoms.
  • Provides hospital discharge and coordination services for clients, which includes, but is not limited to, arranging medically necessary transportation, referring for on-going case management and counseling services, linking with medical and dental care and assist in scheduling follow up psychiatric appointments.
  • Performs basic administrative tasks related to the job, which includes the timely completion of clinical records, forms, and reports.

  • Must be able to form positive therapeutic relationships; to engage clients in the mutual assessment of problems and in goal setting; and to implement follow-up care by assisting in scheduling necessary appointments
  • Must have good oral and written communication skills, ability to make sound decisions under stressful conditions, demonstrated clinical skills, and an ability to organize and prioritize tasks.
  • Must understand and accept a team approach to service delivery and agency and program philosophy and goals.  Must be able to work closely with other team members.
